MS Symptom Graph

An MS symptom graph allows people with MS to easily keep track of their symptoms and the severity of each one.

Use either a spreadsheet or graph paper.

Date:

Write the months along the top of the graph.

Symptoms:

Write all of your symptoms, past and current, as well as new symptoms as they occur, down the left hand side.

Procedure:

On the first of each month rate your symptoms on a scale of 1 (least severe) to 10 (most severe).  Place this number in the square that corresponding with the month and symptom.

Detection at a Glance

By comparing your ratings on the first of each month you can tell at-a-glance if your symptoms are getting better or worse.

Take a copy of this graph with you when you go for your doctor’s visit.
